to be and appear before (the Court) (the State Attorney), at CRIMINAL JUSTICE CENTER, 14250 49TH STREET N., ROOM B200, CLEARWATER, FLORIDA 34618, ON FRIDAY, MARCH 7, 1997 AT 4:00 P.M.

to testify in the above-styled cause and to bring the following described items, to wit:

FURNISH COPY OF ANY AND ALL WRITTEN STATEMENTS PROVIDED TO THE CHURCH OF SCIENTOLOGY BY LAURA R. ARRUNADA DURING THE PERIOD OF NOVEMBER 18, 1995 THROUGH JANUARY 1, 1996.
