FURNISH COPY OF DOCUMENTS REFLECTING CARE AND TREATMENT OF LISA McPHERSON (DOB 02-10-59), SSN 461-94-2009, 901 N. OSCEOLA AVE., #205, BELLEAIR, FLORIDA, TO INCLUDE BUT NOT LIMITED TO DECEMBER 5, AND DECEMBER 6, 1995, AND NOT TO INCLUDE ANY HIV TEST RECORDS GOVERNED BY CHAPTER 381 AND/OR PSYCHIATRIC RECORDS. NOTE: NOTICE HAS BEEN MAILED TO FANNIE McPHERSON AS REQUIRED BY F.S.S. 395.017 AND/OR 455.241.

APPROPRIATE ATTIRE REQUIRED
