INVESTIGATIVE REPORT

0n Friday, November 13, 1998, SA A.L. Strope and Clearwater Detective Sgt. Wayne Andrews, served a Summons to Appear to Attorneys Bob Johnson and Laura Vaughn.

These summons ordered the Church of Scientology Flag Service Organization, Inc. (C.S.F.S.0) to appear before the Circuit Court of the Sixth Judicial Circuit of Florida in and for Pinellas Coun Florida, at the Criminal Justice Center, 14250 49th Street North, Clearwater, Florida on the 30 day of November 1998 at 1:30 p.m. to answer an information filed by the State Attorney for the Sixth Judicial Circuit charging them (C.S.F.S.0) with the crimes of Abuse and/or Neglect of a Disabled Adult and Unauthorized Practice of Medicine.

A copy of the witness affidavit that was also served on C.S.F.S.0 is hereby placed in the 1A section of this case file.
