Dear Mr. Somers:

This agency is currently investigating a death involving suspicious circumstances that occurred on December 05, 1995. The decedent is Lisa McPherson, 901 N. Osceola Avenue, #205, Belleair, Florida.

Our investigation has revealed on November 18, 1995, Lisa McPherson sought medical attention at Morton Plant Hospital, following a motor vehicle accident she was involved in. During the days following her accident, McPherson stayed at a Religious Retreat, located at 210 S. Ft. Harrison Avenue, Clearwater, for rest and relaxation.

On December 05, 1995, McPherson was transported from 210 S. Ft. Harrison Avenue, to Columbia New Port Richey Hospital, where she was pronounced dead shortly after arrival.

In order to establish a medical history profile necessary to our investigation, we are requesting the medical records from Columbia New Port Richey Hospital pertaining to Lisa McPherson, to include the dates of December 5 and 6, 1995.
