FLORIDA 34616

to be and appear before (the Court) (the State Attorney), at CRIMINAL
JUSTICE CENTER, 14250 49TH STREET N., ROOM B200, CLEARWATER, FLORIDA
34618, ON INSTANTER

to testify in the above-styled cause and to bring the following
described items, to wit:
FURNISH LIST OF ALL EMPLOYEES EMPLOYED BY AMC PUBLISHING FROM JANUARY 1,
1995 THROUGH JANUARY 1, 1996, TO INCLUDE FULL LEGAL NAME, CURRENT HOME
ADDRESS, HOME TELEPHONE NUMBER FOR EACH FORMER AND PRESENT EMPLOYEE
WORKING IN THE PERIOD OF JANUARY 1, 1995 THROUGH JANUARY 1, 1996.

APPROPRIATE ATTIRE REQUIRED

NOTE: You are subpoenaed to appear by BERNIE McCABE, STATE ATTORNEY, and
unless excused from this subpoena by this aftomey or the Court, you
shall respond to this subpoena as directed. If you fail to appear, you
may be held in contempt of Court.
