THE STATE OF FLORIDA, TO ALL AND SINGULAR THE SHERIFFS, STATE ATTORNEY
INVESTIGATORS, AND AGENTS OF THE FLORIDA DEPARTMENT OF CRIMINAL LAW
ENFORCEMENT.

YOU ARE HEREBY COMMANDED TO SUBPOENA: DIRECTOR OF LOSS PREVENTION,
MICKEY CARTER, AND/OR AUTHORIZED DESIGNEE, ECKERD DRUG CORPORATION, 8333
BRYAN DAIRY ROAD, LARGO, FLORIDA

to be and appear before (the Court) (the State Attorney), at CRIMINAL
JUSTICE CENTER, 14250 49TH STREET N., ROOM B200, CLEARWATER, FLORIDA
34618, ON INSTANTER

to testify in the above-styled cause and to bring the following
described items, to wit:
FURNISH PRESCRIPTION SUMMARY REPORT FOR DR. DAVID MINKOFF, DEA
#AM1446346, AND ANY AND ALL ORIGINAL PRESCRIPTIONS WRITTEN FOR LISA
McPHERSON AND ANY AND ALL DOCUMENTATION RELATED THERETO, DURING THE TIME
FRAME OF NOVEMBER 1, 1995 THROUGH DECEMBER 30. 1995.

APPROPRIATE ATTIRE REQUIRED

NOTE: You are subpoenaed to appear by BERNIE McCABE, STATE ATTORNEY, and
unless excused from this subpoena by this aftomey or the Court, you
shall respond to this subpoena as directed. If you fail to appear, you
may be held in contempt of Court.
